Field Trip Permission Slip Form Template

A field trip permission slip is a form that gives a parent or guardian's consent for a student to leave school on a trip and collects emergency contacts, medical and allergy notes, and lunch plans so teachers can supervise safely. This template puts the trip details at the top and ends with a typed parent name and consent box, so no slips get lost in backpacks.

Questions

What should a field trip permission slip include?

The destination, date, departure and return times, transport and cost; the student's name and class; whether the child may attend; a parent or guardian contact for the day; allergies, medications and medical notes; lunch arrangements; and a typed parent name with a consent statement.

Are online permission slips accepted instead of paper?

Many schools and districts accept online slips with a typed name and a consent box, but rules differ between districts, states and countries. Have your wording reviewed for your local rules and school policy before you use it.

How do I ask parents to chaperone?

Add a question asking whether they can volunteer, as this template does. Parents who say yes leave a phone number, and you can follow up about any background check your school requires.

Can I reuse this permission slip for every trip?

Yes. Change the trip details text and the due date for each outing. Each trip's replies stay in their own form, so records do not get mixed.

Is this field trip permission slip template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.

Where do the responses go?

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.

Can I host it myself?

Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
