Student Registration Form Template

A student registration form is a form that collects a new student's personal details, grade, parent or guardian contacts, health and learning-support needs, and proof-of-age and address documents so a school can enroll the student and get them ready for day one. This template splits enrollment into four short steps and lets families upload documents instead of bringing paper copies to the office.

Tips for more responses

  1. 1

Families can save progress and come back later, which helps when they need to find a birth certificate or immunization record.

  2. 2

Turn on email notifications so the front office sees each new registration and can follow up on missing documents.

  3. 3

Use a PDF template to produce a filled enrollment record for each student to print or add to the student file.

Questions

What should a student registration form include?

The student's legal name, date of birth and grade; parent or guardian names, relationship and contact details; home address; emergency contact; allergies and medical needs; IEP or 504 status; transportation and care needs; previous school; and uploads for proof of age, proof of address and immunization records.

What documents do schools usually need to register a student?

Typically proof of age such as a birth certificate or passport, proof of residence such as a utility bill or lease, immunization records, and the last report card for transfers. Requirements vary by district and state, so list your own in the form.

Are uploaded documents kept private?

Yes. Files uploaded through the form are stored privately and are not publicly accessible. Only people you give access to the form can see them.

Why ask about an IEP or 504 plan at registration?

Knowing early lets the school request records from the previous school and arrange support before the student starts, so services are not interrupted.

Is this student registration template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.
