Personality Quiz Template

A personality quiz is a form that asks light, scenario-based questions where each answer points to a personality type, so people can find out which type they are and share the result. This template has eight questions with four answers each, labeled A to D, and each letter maps to one type: Adventurer, Planner, Free Spirit or Connector.

Tips for more responses

  1. 1

Results work by majority: the thank-you message lists what 'mostly A, B, C or D' means, so people see their type as soon as they submit.

  2. 2

Add the email plugin with 'recipient from field' to send each person an email copy that includes the type descriptions.

  3. 3

Rename the four types and rewrite the options to match your brand, topic or event; keep each letter tied to the same type in every question.

Questions

What should a personality quiz include?

Four to six clearly different personality types, six to ten everyday scenario questions, one answer per type in every question, a short description of each type, and a way to show the result, such as a thank-you message or a follow-up email.

How are personality quiz results calculated here?

Each answer is labeled A, B, C or D, and each letter belongs to one type. The respondent counts which letter they picked most, and the thank-you message tells them which type that is. Quiz grading is not used, because there are no right or wrong answers.

Is this a scientific personality test?

No. It is a fun, light quiz for engagement and conversation, not a validated psychological assessment like the Big Five. Do not use it for hiring or clinical decisions.

Can I collect emails with a personality quiz?

Yes. The email field is optional here; make it required if the quiz is a lead magnet, and say clearly what you will send so people know why you are asking.

Is this personality quiz template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.

Where do the responses go?

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.

Can I host it myself?

Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
