Trivia Quiz Template

A trivia quiz is a form that asks multiple-choice or short-answer general knowledge questions and scores each answer, so hosts can run a fun competition and players see how much they know. This template has ten tested questions across science, geography, history and the arts, with quiz mode grading every entry automatically.

Tips for more responses

  1. 1

Quiz mode grades every submission and shows the score instantly; the pass mark is set to 70%, so seven correct answers count as a pass.

  2. 2

Set an open and close time window in Form Settings so answers only count during the round, then export to Excel to rank teams.

  3. 3

Turn on 'one response per person' with sign-in for competitive quizzes so nobody resubmits after seeing their score.

Questions

What should a trivia quiz include?

Eight to twelve questions with one clear correct answer each, a mix of topics (science, geography, history, arts, sport), a mix of easy and harder questions, player or team name, and a way to score and show results.

How is the trivia quiz scored?

Each question has an answer key. When someone submits, quiz mode grades their answers automatically, shows their score straight away, and records it with the response so you can sort by score.

Can I write my own trivia questions?

Yes. Copy the template into your free account, replace the questions and options, and set the correct answer for each one. You can also ask the AI form builder to draft a themed round.

How do I stop players looking up answers?

You can't fully prevent it online, but a short open window in Form Settings helps, and so does running the quiz live in a room. Questions that need reasoning rather than a quick search also help.

Is this trivia quiz template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.

Where do the responses go?

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.

Can I host it myself?

Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
