Safety Training Quiz Template

A safety training quiz is a form that tests workers on hazards, safe procedures and emergency steps after training, so employers can confirm understanding and keep a record for each person. This template covers ten core topics, from the PASS fire extinguisher method to lockout/tagout, and grades every answer automatically against an 80% pass mark.

Tips for more responses

  1. 1

Quiz mode shows each worker their score instantly; set the pass mark to match your policy and ask anyone below it to retake after a refresher.

  2. 2

Turn on 'require sign-in' and 'one response per person' so each result is tied to one named worker for your training records.

  3. 3

Use the PDF templates feature to generate a completion record per response, and export results to Excel for audits.

Questions

What should a safety training quiz include?

Questions on the hazards your workers actually face: fire and evacuation, PPE, manual handling, ladders and working at height, chemicals and Safety Data Sheets, lockout/tagout, housekeeping and how to report incidents and near misses, plus the worker's name, ID, site and training date.

Does a safety quiz count as OSHA training documentation?

A quiz can be part of your records, showing the worker was trained and understood it, but it does not replace the training itself or any specific documentation a standard requires. Check the rules that apply to your industry and country.

What pass mark should a safety quiz use?

Many employers use 80% or higher for safety topics, and some require 100% on critical questions like lockout/tagout. Whatever you choose, have a clear retake process.

Can I add questions specific to my site?

Yes, and you should. Copy the template and add questions on your own hazards, emergency exits, assembly points and equipment, each with its correct answer set.

Is this safety training quiz template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.

Where do the responses go?

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.

Can I host it myself?

Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
