Salon Client Intake Form Template

A salon client intake form is a form that collects a new client's hair type, past color and chemical treatments, allergies, scalp sensitivities and the look they want so a stylist can plan the service and run any needed patch tests before the client sits in the chair. This template asks the questions that change a color formula or rule out a relaxer, and lets clients upload inspiration photos so the consultation starts from a picture, not a guess.

Tips for more responses

  1. 1

Use conditional logic to ask about past reactions only when a client says they have had one.

  2. 2

Turn on email notifications so the stylist on the booking sees the inspiration photos the night before.

  3. 3

Use a PDF template to print each intake as a client record card that sits with the color formula.

Questions

What should a salon client intake form include?

Contact details, hair type and texture, natural and current color, color and chemical treatments in the last two years, use of box dye or henna, allergies and past reactions to hair products, scalp conditions, daily styling routine and heat use, the service and look they want, inspiration photos, and consent for a patch test.

Why do salons ask about box dye and henna?

Home dyes and henna can react unpredictably with lightener and professional color, causing uneven results or damage. Knowing about them in advance lets the colorist plan a strand test or change the service.

Do I need a patch test before coloring hair?

Many color manufacturers recommend a skin allergy test 48 hours before every oxidation color service, and some insurers require it. Follow the instructions on the products you use and your insurer's terms.

Can clients send inspiration photos with the form?

Yes. The form has an upload field for up to five photos, stored privately with the client's answers.

Is this salon client intake template free?

Yes. Copy it into a free Dculus Forms account, change anything you like and start collecting responses. No credit card is needed.

Can I change the questions?

Yes. Your copy opens in the form builder, where you can add, remove and reorder questions, change the layout and add your own branding.

Where do the responses go?

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.

Can I host it myself?

Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
