Liability Waiver Template
A liability waiver is a form in which a participant acknowledges the risks of an activity, releases the organizer from certain claims, and gives an emergency contact, so the organizer has a record of informed agreement before the activity starts. This template uses plain-language sample terms, a typed full name and a single consent box, and it covers adults signing for themselves or a parent signing for a minor.

Tips for more responses
- 1
Treat the waiver text as a starting point and have it reviewed by a lawyer for your local rules before you use it.
- 2
Show the waiver as a QR code at check-in so participants complete it on their phone before the activity.
- 3
Turn on a PDF template to create a filled waiver PDF for each response, and send the participant an email copy.
Questions
What should a liability waiver include?
The participant's name, date of birth and contact details; the activity and date; a clear description of the risks; an assumption of risk and release of claims; permission for emergency medical treatment; an emergency contact; and the participant's typed name, date and agreement. For minors, add the parent or guardian's details.
Is a typed name on an online waiver binding?
In many places a typed name plus a clear consent box can show agreement, but whether a waiver holds up depends on local law, the wording and the activity. Waivers usually cannot excuse gross negligence. Have yours reviewed by a lawyer for your local rules.
Can a parent sign a waiver for a child?
Parents commonly complete waivers for minors, but courts in some states and countries do not enforce a parent's release of a child's future claims. Check your local rules and insurer's guidance.
How long should I keep signed waivers?
Many organizers keep them at least as long as the time limit for injury claims where they operate, often several years and longer for minors. Responses stay in your account, and you can export them to Excel or CSV for your records.

Responses arrive in your Dculus Forms workspace. You can export them to Excel or CSV, send them to Google Sheets or a webhook, and get email notifications.
Can I host it myself?
Yes. Dculus Forms is open source (GPLv3), so you can run it on your own servers and keep every response in your own database.
